If you would like to baptize your child at OLQA, please read the Baptism Guidelines (click button below) and fill out the Baptism Application Form. Please make sure to click on the “submit” button at the end. Once that has been submitted, the office staff will be in touch with you to continue the process.

Pre-Baptism classes are required for parents who have not previously baptized a child and are held on the first Wednesday of each month at 6:30 pm. Registration is required.
